Job Description
In this remote role, you’ll be the bridge between enrollment and engagement—ensuring that families who’ve said “yes” to support are smoothly transitioned through the multiple steps of the GUIDE application process. This isn’t just a job – it’s a chance to help caregivers feel confident, informed, and supported as they begin a journey that could transform their caregiving experience.
What You’ll Do
Guide patient and their caregivers through the onboarding process after enrollment
Monitor completion of dementia screening and health assessments
Coordinate tablet shipment and provide orientation to the GUIDE program
Schedule and track formal neurology evaluations with our clinical partners
Review documentation and prepare applications for CMS submission
Track progress and report status updates to internal teams
Ensure a warm handoff to Engagement Specialists and Coaches
Collaborate on software workflows
Adapt to evolving processes and contribute to continuous improvement
