Cherry Brook Health Care Center, a division of New Horizons, Inc., is a 5 star rated 100 bed skilled nursing facility in Canton, CT. Our facility has spacious private and semiprivate rooms, a well-appointed dining area, comfortable lounges and a beautiful landscaped outdoor terrace.

PURPOSE OF YOUR POSITION:

The primary purpose of your job position is to provide direct nursing care under the supervision of the RN to residents and to supervise the day-to-day nursing activities performed by the nursing assistant. Such supervision must be in accordance with current federal, state, and local standards, guidelines, and regulations that govern our facility, and as may be required by the RN to ensure that the highest degree of quality care is maintained at all times.

RESPONSIBILITIES INCLUDE:

  • Meet the nursing and rehabilitation needs of each resident.
  • Maintain current resident care plans on all residents under his/her care.
  • Follow physician’s orders. Renders and/or assists with complete nursing care, treatments, dressings, and other physician orders.
  • Review patient records daily to assure accuracy and completeness.
  • Document comprehensive and complete nursing notes.
  • Document and report any unusual or significant findings; contact the physician when necessary.
  • Administer medication according to procedure.
  • Maintain an accurate and safe control of all drugs for which he/she is responsible.
  • Complies with facility Privacy Practices Policies & Procedures, protecting residents’ individual health information.
  • Demonstrate leadership skills.
  • Fully complies with all federal and state laws and standards of ethical and professional conduct applicable to their position.
  • Knowledge about laws and regulations relating to job responsibilities, e.g., EEO, OSHA, patient rights, fraud/abuse laws.

EDUCATION:

Must have an LPN license in good standing in CT. Must be CPR certified.

EXPERIENCE:

  • A minimum, one (1) year experience in a hospital, long-term care facility or other related health care facility is preferable.

SPECIFIC REQUIREMENTS:

  • Must possess a current, unencumbered license to practice as an LPN in this state.
  • Must be able to read, write, speak and understand the English language.
  • Must be knowledgeable of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to long-term care.
  • Must possess leadership and supervisory ability and the willingness to work harmoniously with professional and non-professional personnel.
  • Must possess the ability to plan, organize, develop, implement, and interpret the programs, goals, objectives, policies and procedures, etc., that are necessary for providing quality care.
  • Must possess accurate and comprehensive assessment skills.
  • Must have knowledge of restorative nursing rehabilitation policies and utilizes appropriately.
  • Must be able to communicate effectively to appropriate personnel regarding emergencies.
  • Must be able to push, pull, and do heavy lifting without restrictions up to 51 pounds.
  • Must be able to assist in the evacuation of residents in emergencies.
