Job Description

Ensures care continuity for patients and families and collaborates in the development, implementation, and evaluation of all program components. Develops, implements, and enhances special projects promoting inpatient/outpatient continuity of care and parent engagement. Ensures the provision of high quality, individualized, comprehensive, cost effective patient care across the continuum. Initiates, tracks, analyzes, and manages all program quality improvement and research projects, program goals and objectives. Coordinates functions with interdisciplinary medical care, liaison functions with patient, family, medical staff, and referring physicians. Develops educational materials and programs for patients, nursing and ancillary staff, and physicians. Develops clinical protocols, policies, and procedures including community education and public relations. Education Requirement: Bachelor's Degree required; Masters Degree in Nursing preferred. Licensure Requirement: Registered nurse in the State of Ohio required. Skills: Effective customer service skills, good multiplatform communication skills (oral, written, presentation), ability to assist in conflict resolution, problem-solving and interpersonal skills required for interaction with contacts. Experience: Three years of registered nurse experience required. Physical Requirements: Occasionally standing, walking; frequently decision making; continuously audible speech, computer skills, hand use (grasping, gripping, turning), hearing acuity, interpreting data, peripheral vision, problem solving, repetitive hand/arm use, seeing (far/near), sitting.
