Job Title: MDS Coordinator
Facility: Bedrock Rehabilitation and Nursing Center at Vista Manor
Location: Titusville, Florida
Shift: Full-Time
Pay Rate: Up to $41 per hour

Position Overview:
Join our compassionate team at Bedrock Rehabilitation and Nursing Center at Vista Manor as an MDS Coordinator, where you'll play a key role in driving exceptional resident care through expert assessments, personalized care planning, and interdisciplinary collaboration. Key Responsibilities:

Promote Quality Care: Ensure our residents receive the highest level of care by maintaining and improving clinical standards.

Conduct Resident Assessments: Perform comprehensive MDS assessments and interviews to create individualized, goal-driven care plans.

Develop Strategic Care Plans: Design care strategies that promote both short-term recovery and long-term wellness.

Foster Team Collaboration: Lead care planning meetings with healthcare providers, residents’ families, and caregivers to ensure a cohesive approach.

Support Positive Outcomes: Partner with specialists and internal teams to enhance resident health and satisfaction.

Lead & Educate: Guide staff through in-service training sessions on MDS, RAPS, and Triggers, and chair nursing management meetings to keep the team informed and aligned.

Ensure Regulatory Compliance: Maintain accurate MDS schedules and documentation for non-Medicare residents.

Requirements:

Current Registered Nurse (RN) license in the state of Florida

At least 3 years of experience in a Skilled Nursing Facility (SNF)

In-depth knowledge of MDS, Medicare, and the care planning process
